Frequently Asked Questions
New to psychiatric care, or just new to The Inner Piece? Here are the questions we hear most — about getting started, medication, telehealth, insurance, and cost. If you don't see your question here, call us at (313) 710-8070 and we're happy to help.
Getting Started
-
Call us at (313) 710-8070 or reach out through our contact page. We'll talk through what you're looking for, confirm insurance or payment details, and schedule your initial psychiatric evaluation — in person at our Farmington Hills office or virtually, anywhere in Michigan.
-
Your first visit is a full psychiatric evaluation (about 60 minutes, $200). Your provider will listen to what you've been experiencing, review your history and current medications, and talk through your goals. From there, we build a plan together — which may include medication, therapy, or both — and explain it in plain language, so decisions about your care are shared decisions.
-
A photo ID, your insurance card, and a list of your current medications (including doses) and any supplements. If you have records from a previous provider, bring those too if you can. It also helps to jot down your main symptoms and questions ahead of time — but you don't need to have it all figured out before you come in.
-
A psychiatric nurse practitioner (PMHNP-BC) is an advanced practice nurse with specialized graduate training in mental health. We can evaluate and diagnose mental health conditions, prescribe and manage psychiatric medication, and provide psychotherapy. At The Inner Piece, medication management and talk therapy are available in one place.
-
Yes. We treat patients age 6 and up — children (6–10), teens, and adults. Both Monica Goedert, PMHNP-BC and Dr. Brittany Sager, DNP, PMHNP-BC see patients ages 6 and older.
Treatment & Medication
-
No. An evaluation doesn't commit you to medication. Some people do best with therapy alone, some with medication, and many with both. We'll talk through your options, the benefits and risks of each, and what fits your goals and your life — the choice is yours.
-
Medication management is the follow-up process after your evaluation: shorter appointments (about 20 minutes, $130) where we check how you're responding, watch for side effects, and adjust doses or medications when needed. How often we meet depends on your situation — more often when you're starting or changing a medication, and less often once you're stable.
-
It varies. Many medications — antidepressants, for example — take two to six weeks before you notice a meaningful difference, and the full effect can take longer. Your provider will tell you what to expect from your specific medication, and what to do if something doesn't feel right along the way.
-
Honestly, we don't know yet — and we'd rather be straight with you than pretend otherwise. Some people need medication for a "season," while they get through a hard stretch. Others find they need it longer than they expected — and if that's the case, we believe quality of life is worth it. What we can promise is that medication here is never "set and forget": we continuously reassess your goals together and make adjustments as things change — including lowering a dose or stopping when the time is right.
-
Tell us — that's a conversation, not a conflict. Just don't stop suddenly on your own. Many psychiatric medications need to be tapered down gradually; stopping abruptly can cause uncomfortable discontinuation symptoms (like dizziness, flu-like feelings, irritability, or "brain zaps") and can bring back the very symptoms you've worked hard to improve. When it's time to come off a medication, we build a slow, safe weaning plan together and check in as you go.
-
Yes — that's exactly how we're set up. The Inner Piece offers supportive talk therapy (45 minutes, $150) alongside medication management. And we share Suite G with our partner practice, Working Through Therapy Group, so patients who want in-depth talk therapy can coordinate both kinds of care under one roof.
-
We work with children, teens, and adults across a range of mental health concerns — including anxiety, depression, and mood concerns — and we have a special focus on women's mental health, including life after having a baby. If you're not sure whether we can help with what you're facing, call us and ask.
Telehealth & Location
-
Yes. In addition to in-person visits in Farmington Hills, we offer telehealth appointments to patients age 6 and up anywhere in Michigan. You'll need a phone, tablet, or computer with a camera and a private space. Because of licensing rules, you do need to be physically in Michigan during your virtual appointment.
-
The Inner Piece is at 31400 Northwestern Highway, Suite G, Farmington Hills, MI 48334 — in the same suite as Working Through Therapy Group.
Insurance & Fees
-
We are in-network with Aetna, ASR Health Benefits, Blue Care Network, BlueCross and BlueShield, Cigna and Evernorth, McLaren, Medicaid, Medicare, Optum, and Priority Health. Coverage varies by individual plan, so we encourage you to verify your mental health benefits with your insurance company before your first appointment.
-
If you have a PPO plan, you may be able to submit for out-of-network reimbursement. Call us and we can talk through your options.
-
Initial psychiatric evaluation (60 minutes): $200. Medication review / follow-up (20 minutes): $130. Supportive talk therapy (45 minutes): $150. We also offer a sliding scale — ask us if cost is a barrier to getting started.
-
Cash, check, PayPal, and Zelle.
Good to Know
-
We require 48 hours' notice to cancel or reschedule an appointment. Appointments cancelled with less than 48 hours' notice — or missed without notice — are charged a $50 fee.
-
Most refills are handled at your follow-up appointments. If you need a refill between visits, ask your pharmacy to send us a refill request, or call us at (313) 710-8070, at least 3 business days before you run out — refills are reviewed and approved during regular office hours. If you're overdue for a follow-up, or a medication needs monitoring or labs, we may ask you to schedule an appointment before refilling. To avoid gaps, a good rule is to request your refill when you still have a week's worth of medication left. Please note: controlled substances will not be refilled early.
-
Yes. What you discuss in your care is kept private, with the specific exceptions required by law — such as imminent risk of harm or suspected abuse or neglect. We'll review confidentiality and its limits with you at your first appointment.
-
The Inner Piece does not have after-hours coverage. Messages left after hours are returned on the next business day. If you or someone you love is in crisis or it's an emergency, call or text 988 (the Suicide and Crisis Lifeline), call 911, or go to your nearest emergency room.
